Most of Granger Medical physicians participate with all major insurance companies; see a representative list below. We cannot guarantee all of our providers accept all these companies and all levels of their coverage. If you have questions regarding plan participation, contact your insurance company directly. Granger Medical’s accounting system is set up for “individual accounting,” which means all patients are set up under their own accounts. All patients over age 18 will be listed as their own guarantors (responsible for the account). Each person receives a separate statement for their account with the exception of patients under age 18, who will be listed on the same statements as their guarantors.
Your insurance card, photo identification, and co-payments are required at the time of service each time you visit our clinics.
Laboratory or diagnostic services cannot always be performed internally. When processed by an outside entity, you will receive a separate bill for these services.
