Insurance Information

Granger Medical Clinic providers accept most insurance plans, which enables patients to remain with Granger Medical Clinic and receive care even if insurance coverage changes. Commonly accepted insurance plans include:

  1. United Healthcare
  2. PEHP
  3. Select Health
  4. Humana
  5. Altius
  6. Blue Cross
  7. Aetna

New Patients

All new patients should plan to arrive early to complete the registration process. When checking in, please be ready to provide the following information:

  1. Photo identification
  2. Current insurance card(s)
  3. Copayment (as necessary)
  4. List of medications and allergies
  5. Referral (as necessary)
  6. CT Scans
  7. MRI reports
  8. X-rays
  9. Medical and operation records from past doctor appointments

Note: Patients with high deductible plans may be asked to pay $75 at the time of visit, which will be applied towards the total bill.

Payments and fees

Your Granger Medical Clinic healthcare team works hard to continuously reduce the cost of medical care. When you make payments for services at the time you receive them, you play a big part in supporting our efforts to keep our costs low.

All Granger Medical Clinic payments can be made using credit cards, checks or cash.

We are more than happy to work with you in handling claims as much as possible, including billing your accepted insurance provider on your behalf. However, it is important to remember that payments are ultimately the patient’s responsibility.

Renewals and prescriptions

Please request all prescription refills or other authorizations during normal office hours, when all medical records can be accessed. If you make a request outside of normal business hours, it will only be filled under emergent or extreme circumstances. Requests can be denied for many different reasons so it is important to make your request prior to completely running out of a medication.

Most prescription refill requests will take at least 24 hours to process.

Online Payment Option

Certain Granger Medical Clinic providers accept online payments. To make a payment online, you’ll need your:

  1. Patient ID (use the Account # listed on your statement)
  2. First name, last name and date of birth
  3. Email address (payer’s)
  4. Payment amount
  5. Credit/debit card, or bank routing and account number
  6. You do not have to be enrolled in the Patient Portal to make a payment online.

Other Payment Options

Payments can also be made in person at any Granger Medical Clinic location, over the phone, or by mail. For questions regarding the online payment process, arranging payment options, an account in collections, or if you would like to make a payment over the phone, contact our billing department using the information below.

Release of Medical Records

In compliance with the Health Insurance Portability and Accountability Act 1996 (HIPAA) and our privacy policy, Granger Medical Clinic must receive a written request in order to release any medical records from our clinic. Granger Medical Clinic may not release records for outside institutions (such as hospitals or other clinics).

Request Your Records

All record releases are processed through Granger Medical Clinic’s Records Office in our West Valley location. To request your records, please download and complete the Records Release form and bring it with you to our Medical Records office along with valid photo identification.

If the form is filled out completely and there is no need for additional information, you can expect to receive your medical records within 7-10 days but no later than 30 days after the request. There may be a fee associated with a medical records release.
