Granger Direct Care and Wellness

Our Primary Care Membership is ideal for children and adults seeking preventive care, routine sick visits, and management of common chronic conditions – all with the
exceptional access and service of the DPC model. This plan covers the full. scope of
primary care services for one affordable monthly fee:

• Comprehensive Primary Care: Preventive wellness exams, routine screenings, and management of everyday illnesses or injuries are all included. We also help manage chronic diseases like diabetes, high blood pressure, asthma, and more – with close monitoring and follow-ups to keep you healthy.
• Basic Procedures & ln-Office Tests: Minor procedures such as wound care, stitches, EKG heart tracings, breathing treatments, etc., that are done in-office and performed by Brittany are included at no extra charge. Any services or testing need that Brittany can’t personally provide can still be done on site through Granger Medical in-house services, including Labs, X-ray, CT, MRl, Vaccines, Cortisone injections, IUD placement, and lV services can be done through your insurance – reducing the need for outside referrals.
• Mental Health Support: We offer basic behavioral health support, counseling for anxiety or depression, and care coordination for therapy or specialty care as needed – recognizing that mentaI wellness is part of primary care.
• Telehealth & Messaging: can’t make it in person? Members enjoy telehealth consults, phone calls, and secure messaging with Brittany during office hours. Get your questions answered and care coordinated from the comfort of home. Afterhours access is through our department clinic on call group.
• Speciatist Coordination: If you do need a specialist (e.g. cardiologist, neurologist, etc.), we will coordinate referrals and work closely with them. Brittany will help navigate outside tests or treatments, so nothing falls through the cracks.
• Discounts for Procedures & Services at Granger not included in DPC: Labs, Xray, Mammogram, CT Scan, MRl, Vaccines, Fracture Casting, IUD placement, Cortisone injections, lV placement and lV fluid administration by Dr. Coleby’s RN are eligible for a discount (or insurance coverage if requested.) Services typically not covered by insurance include Botox, PRP Joint injections, lV Infusions of vitamins or NAD+ are available at a discount.

You’ll never feel rushed – we schedule longer visits to address all your concerns. 90% of healthcare needs can be managed by quality primary care, and we strive to handle as much as possible for you in-house. Note: The Primary Care membership is perfect for most routine healthcare
needs. If you have a complex chronic illness*** that requires more intensive
coordination or specialty expertise, you can upgrade to our Specialty Care plan at
any time. We’re here for you no matter your health journey. A Specialty Care plan
is required if your complex chronic illness is listed below.

Our Specialty Care Membership is an enhanced plan for patients with rare, complex,
or multi-system medical conditions who need a higher level of support. This tier includes everything in the standard primary care membership plus additional time, expertise, and care coordination for complicated health issues (sometimes called a “medical home” model for complex chronic disease). If you or your loved one is dealing with conditions tike POTS, EDS, ME/CFS, Long COVID, MCAS or other challenging diagnoses, this plan is designed for you.

What’s Included: All Primary Care services plus+:

• Extended Visits: No limits on appointment length or frequency – schedule as many visits as needed, with longer visit times available. Complex patients often need 1-hour visits; we make that possible so we can truly listen, review records, and form a comprehensive plan. There are zero co-pays or per-visit charges for these appointments.
• ln-Depth Care Coordination: We become your healthcare quarterback. Brittany will coordinate with your specialists (cardiology, neurology, rheumatology, etc.) to ensure everyone is on the same page. We help arrange referrals, follow up on specialist recommendations, and even facilitate communication among providers. Our team has extensive experience managing multi-system conditions and will help orchestrate the complex puzzle of your care.
• Personalized Education & Support: You’lI receive personalized education about your condition and guidance on management strategies. We take time to explain the “why” behind treatments and provide up-to-date information on your illness. Our clinic is a safe space where your concerns are heard and validated – no more feeling dismissed or “it’s all in your head.”
• Advocacy and Navigational Help: Managing rare diseases can be overwhelming. We offer ongoing support and advocacy, helping you navigate insurance hurdles, obtain necessary tests or therapies (appeals, letters of medical necessity), and connecting you with patient resources. We’re your partner in this journey, every step of the way.
• Collaborative Gare Plan: Brittany works in consultation with Dr. Coleby**** and a network of specialists to craft and continually update a comprehensive care plan tailored to you. We focus on managing symptoms, improving quality of life, and maintaining optimal function. Our goat is to keep you as healthy and active as possible, coordinating everything from medications to lifestyle interventions across all your providers.

**** ln person visits with Dr. Coleby are not part of the DPC plan. However, if
needed, Brittany may consult with him and arrange a comprehensive
evaluation through his Autonomic Disorders Clinic. She will then continue your
care based on that plan. Appointments with Dr. Coleby require either insurance
coverage or a separate cash payment. Although his clinic has been closed to
new patients since 2022, he is now seeing new patients on a case-by-case basis
if Brittany is established as their primary care provider and is the referring
provider.

Conditions We Support: We welcome patients with diagnoses (or even undiagnosed
conditions) such as Postural Orthostatic Tachycardia Syndrome (POTS), Ehlers-Danlos Syndrome (EDS), Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS), Long COVID, Mast Cell Activation Syndrome (MCAS), autoimmune dysautonomia and related disorders, fibromyalgia, small fiber neuropathy, autonomic dysfunction, vascular compression syndromes (MALS, SMAS, Nutcracker, May-Thurner), orthostatic intolerance and more. If you have a complex condition not getting the attention it deserves, we are here to help with cutting-edge knowledge and compassionate care. (Our clinic works alongside your specialists – we do not replace cardiologists, neurologists, etc., but ensure all care is well-coordinated and nothing is overlooked.)

• Brittany Jorgensen, FNP-C – Lead Provider: Brittany is a Board-Certified Family Nurse Practitioner with over 18 years of nursing experience. She spent 15+ years as an RN (including surgical and trauma care) before earning her Master’s and FNP certification in 2018. As a primary care provider, she grew frustrated with rushed, high-volume clinic settings that limited her time with patients. This led her to join the renowned Metrodora Institute in 2023, a multidisciplinary clinic for complex neuroimmune disorders, where she gained extensive experience caring for patients with conditions tike POTS, autoimmune dysautonomia, and EDS in collaboration with top specialists. Now, Brittany brings that expertise to Granger Direct Care & Wellness. She is known for her empathetic listening, thorough approach, and dedication to helping patients who have “fallen through the cracks” of the healthcare system. Brittany will be your primary partner in health – whether you’re coming in for a routine physical or navigating a rare illness, you will be in capable and caring hands.
• Craig Coleby, MD – Medical Director: Dr. Coleby is a highly accomplished physician providing oversight and medical director support at our clinic. He is triple board-certified in Internal Medicine, Pediatrics, and Sports Medicine, with an additional subspecialty certification in Autonomic Disorders (Neurology subspecialty). A Utah native, Dr. Coleby has been with Granger Medical Clinic since 2010 and has 3,000+ hours of emergency and critical, care experience. He has a special interest in complex conditions that affect the autonomic nervous system (like POTS and related syndromes)and helps organize the annual “POTS and Beyond” physician education conference at the University of Utah. Dr. Coleby is an Adjunct Assistant Professor in the Department of Education at the university of Utah. As MedicaI Director, Dr. Coleby ensures that our clinic provides the highest quality of care. He’ll work closely with Brittany on her approach to challenging care, bringing his broad expertise in adult and pediatric medicine. Patients appreciate Dr. Coleby’s down-to-earth demeanor and problem-solving mindset. Together, he and Brittany are excited to offer an innovative care model that puts patients first.

What’s Included in the Membership?

• Unlimited office visits (in person or telehealth)

• Complex questions will be addressed in a visit, allowing time for discussion, documentation and update to your current treatment plan
• Same- or next-day appointments for urgent concerns or complex questions
• 30- 60-minute appointments to address your questions without feeling rushed
• Care coordination with your other providers
• Point-of-care testing

Can I use my medical insurance?

• We welcome patients with any insurance plan—or no insurance at all.
• We do not bill insurance for our services.
• Your direct primary care is fully covered by your monthly membership fee.
• If you have insurance, you can still use it for labs, medications, imaging, specialist visits, and hospital services.

What if I don’t have health insurance?

• No problem! Your monthly membership covers all in-office visits, secure messaging, and point-of-care testing, with no additional copays or out-of-pocket charges.
• You can be seen as often as needed each month at no extra cost.
• We recommend pairing your membership with a health insurance plan to help cover prescriptions, labs, imaging, specialist referrals, emergencies, and hospitalizations. Many patients choose a high-deductible plan alongside their membership.

Do you accept Medicare patients?

• We are not able to accept Medicare patients at this time.
• With ongoing changes to Medicare reimbursement, this may change in the future.
• If you’d like, we can add you to our waitlist and contact you if this restriction changes.

Can I use my HSA or FSA to pay for the monthly membership fee?

• FSA: Yes, you can use your FSA funds for the membership without any issues.
• HSA: Many HSA plans allow the use of funds for medical services; however, please check with your HSA administrator to confirm whether your plan permits HSA funds to be used for our membership services.

Is a Direct Care Practice (DCP) the same as a Concierge Practice?

• Direct Care Practice (DCP) takes the best parts of the concierge concept – unprecedented access to your provider and a focus on personalized care – and makes them accessible to more people. Concierge practices however, charge a high annual membership fee in addition to billing your health insurance plan for your visits and procedures. This means in addition to the high-cost annual fee, you are still responsible for copays, deductibles, and other costs dictated by your insurance. A concierge practice often has 24/7 access and is tailored to the wealthy.
• DCP allows direct access to your provider, with same day or next day availability. While this is not 24/7 access, the fast appointments, direct communication with your provider and proactive medical care make a significant difference in your health and well-being.
• To ensure high-quality, accessible care, Brittany Jorgensen FNP maintains a limited patient panel, balancing complex chronic disease management with primary direct care so each patient receives adequate time and attention each month.
