Granger Direct Care & Wellness

The Primary Care Membership is designed for children and adults seeking preventive care, sick visits, ongoing management of common chronic conditions, and a more convenient, patient-centered primary care experience.

This membership includes comprehensive primary care, routine screenings, chronic disease management, telehealth access, behavioral health support, specialist coordination, and select in-office procedures—all delivered with more time, access, and continuity than traditional care models.

What’s Included

Comprehensive Primary Care

Your membership covers preventive wellness exams, routine screenings, and care for everyday illnesses and injuries. We also provide ongoing management for chronic conditions such as diabetes, high blood pressure, asthma, and more—with close monitoring and follow-up to help you stay on track and avoid complications.

Basic Procedures & In-Office Testing

Many common in-office services are included at no additional cost when performed by Brittany, including:

• Wound care and stitches
• EKG (heart tracings)
• Breathing treatments

If additional services are needed, Granger Medical’s in-house resources make it easy to stay within one system. Services such as labs, X-ray, CT, MRI, vaccines, cortisone injections, IUD placement, and IV services can be completed on-site and billed through your insurance—reducing the need for outside referrals.

Mental Health Support

We recognize that mental health is a critical part of overall wellness. Your membership includes support for common conditions such as anxiety and depression, along with care coordination for therapy or specialty mental health services when needed.

Telehealth & Direct Communication

Can’t make it into the clinic? No problem. Members have access to:

• Telehealth visits
• Phone consultations
• Secure messaging during office hours

This allows you to get answers, adjust treatment plans, and stay connected to your care—without always needing an in-person visit. After-hours needs are supported through Granger’s on-call provider group.

Specialist Coordination

If specialty care is needed, we help guide the process from start to finish. Whether you’re seeing a cardiologist, neurologist, or another specialist, Brittany will coordinate referrals, assist with testing, and ensure your care remains connected and well-managed.

Discounts on Additional Services

Members may receive discounts on select services within Granger Medical that are not included in the membership. These may include:

• Labs, imaging (X-ray, CT, MRI), and mammograms
• Vaccines and fracture care
• IUD placement and cortisone injections
• IV placement and IV fluid administration

Services typically not covered by insurance—such as Botox, PRP joint injections, and vitamin or NAD+ IV infusions—may also be available at a discounted rate.

Approximately 90% of healthcare needs can be effectively managed through high-quality primary care, and our goal is to handle as much of your care as possible in-house—saving you time, reducing complexity, and improving outcomes.

Primary Care Membership Pricing

Monthly pricing for individual members is age-based:

• Ages 0–30: $100/month
• Ages 31–60: $125/month
• Ages 61+: $150/month
• One-time enrollment fee: $100

Family pricing is also available:

• Children ages 0–17: $50/month each
• Spouse/partner add-on example for ages 31–60: $75/month
• Two parents and two children: $200/month
• Additional children: $50/month each
• Family membership is capped at $350/month
• Family enrollment fee: $250

While our Primary Care Membership is designed to meet the needs of most patients, some individuals require a higher level of support, coordination, and clinical expertise.

If you are living with a complex chronic illness, you can upgrade to our Specialty Care Membership at any time. This advanced care model is built specifically for patients navigating multi-system, rare, or difficult-to-manage conditions—and ensures you receive the time, attention, and coordination your care requires.

Conditions Requiring Specialty Care Membership

Patients with the following conditions require enrollment in the Specialty Care plan due to the complexity and level of care coordination involved:

Complex & Undiagnosed Conditions

• Rare or undiagnosed medical conditions (“medical mystery” cases)

Autonomic & Neurological Disorders

• Postural Orthostatic Tachycardia Syndrome (POTS)
• Orthostatic Intolerance (OI) not responsive to neurology or cardiology treatment
• Autonomic disorders and dysfunction, including:
  • Refractory vasovagal syncope
  • Autonomic neuropathy
  • Autonomic ganglionopathy
  • Autonomic failure
  • Multiple System Atrophy (MSA)

Connective Tissue & Chronic Fatigue Conditions

• Ehlers-Danlos Syndrome (EDS)
• Hypermobility Spectrum Disorders (HSD)
• Myalgic Encephalomyelitis / Chronic Fatigue Syndrome (ME/CFS)

Post-Viral & Chronic Pain Conditions

• Long COVID
• Fibromyalgia
• Complex Regional Pain Syndrome (CRPS)

Autoimmune & Systemic Conditions (Refractory Cases)

• Autoimmune complications related to:
  • GAD-65
  • Sjögren’s Syndrome
  • Lupus
  • Rheumatoid Arthritis
  • Scleroderma
    (particularly when not adequately managed through rheumatology)

Neurological & Pain Disorders

• Small Fiber Neuropathy
• Chronic complex migraines not responsive to neurology treatment

Intracranial & Headache Disorders

• Orthostatic headaches, including:
  • Spontaneous Intracranial Hypotension (SIH)
  • Idiopathic Intracranial Hypertension (IIH) / Pseudotumor Cerebri

Immunologic & Mast Cell Disorders

• Mast Cell Activation Syndrome (MCAS)

What’s Included in the Specialty Care Membership?

As a Specialty Care Member, you will recieve all care available under the Primary Care Membership plus:

• Extended Visits: No limits on appointment length or frequency – schedule as many visits as needed, with longer visit times available. Complex patients often need 1-hour visits; we make that possible so we can truly listen, review records, and form a comprehensive plan. There are zero co-pays or per-visit charges for these appointments.
• ln-Depth Care Coordination: We become your healthcare quarterback. Brittany will coordinate with your specialists (cardiology, neurology, rheumatology, etc.) to ensure everyone is on the same page. We help arrange referrals, follow up on specialist recommendations, and even facilitate communication among providers. Our team has extensive experience managing multi-system conditions and will help orchestrate the complex puzzle of your care.
• Personalized Education & Support: You’lI receive personalized education about your condition and guidance on management strategies. We take time to explain the “why” behind treatments and provide up-to-date information on your illness. Our clinic is a safe space where your concerns are heard and validated – no more feeling dismissed or “it’s all in your head.”
• Advocacy and Navigational Help: Managing rare diseases can be overwhelming. We offer ongoing support and advocacy, helping you navigate insurance hurdles, obtain necessary tests or therapies (appeals, letters of medical necessity), and connecting you with patient resources. We’re your partner in this journey, every step of the way.
• Collaborative Care Plan: Brittany works in consultation with *Dr. Coleby and a network of specialists to craft and continually update a comprehensive care plan tailored to you. We focus on managing symptoms, improving quality of life, and maintaining optimal function. Our goat is to keep you as healthy and active as possible, coordinating everything from medications to lifestyle interventions across all your providers.

* ln person visits with Dr. Coleby are not part of the DPC plan. However, if
needed, Brittany may consult with him and arrange a comprehensive
evaluation through his Autonomic Disorders Clinic. She will then continue your
care based on that plan. Appointments with Dr. Coleby require either insurance
coverage or a separate cash payment. Although his clinic has been closed to
new patients since 2022, he is now seeing new patients on a case-by-case basis if Brittany is established as their primary care provider and is the referring
provider.

What Is Not Included?

Some services fall outside the monthly membership and remain the patient’s financial responsibility. These include:

• Emergency room and inpatient hospital services
• Specialist care or outside facility services
• Medications, vaccines, imaging, and labs performed outside the practice
• Certain self-pay or cash-pay procedures outside basic in-office care, such as PRP injections or iron infusions

Members may receive discounts on select Granger services and some procedures not included in the membership. The website also notes that certain in-house services and procedures may be eligible for discounts or insurance billing, depending on the service.

Our compassionate team is what truly sets Granger Direct Care and Wellness apart. We combine decades of experience in primary care and specialty medicine with a genuine passion for patient-centered care.

• Brittany Jorgensen, FNP-C – Lead Provider: Brittany is a Board-Certified Family Nurse Practitioner with over 18 years of nursing experience. She spent 15+ years as an RN (including surgical and trauma care) before earning her Master’s and FNP certification in 2018. As a primary care provider, she grew frustrated with rushed, high-volume clinic settings that limited her time with patients. This led her to join the renowned Metrodora Institute in 2023, a multidisciplinary clinic for complex neuroimmune disorders, where she gained extensive experience caring for patients with conditions tike POTS, autoimmune dysautonomia, and EDS in collaboration with top specialists. Now, Brittany brings that expertise to Granger Direct Care & Wellness. She is known for her empathetic listening, thorough approach, and dedication to helping patients who have “fallen through the cracks” of the healthcare system. Brittany will be your primary partner in health – whether you’re coming in for a routine physical or navigating a rare illness, you will be in capable and caring hands.
• Craig Coleby, MD – Medical Director: Dr. Coleby is a highly accomplished physician providing oversight and medical director support at our clinic. He is triple board-certified in Internal Medicine, Pediatrics, and Sports Medicine, with an additional subspecialty certification in Autonomic Disorders (Neurology subspecialty). A Utah native, Dr. Coleby has been with Granger Medical Clinic since 2010 and has 3,000+ hours of emergency and critical, care experience. He has a special interest in complex conditions that affect the autonomic nervous system (like POTS and related syndromes)and helps organize the annual “POTS and Beyond” physician education conference at the University of Utah. Dr. Coleby is an Adjunct Assistant Professor in the Department of Education at the university of Utah. As MedicaI Director, Dr. Coleby ensures that our clinic provides the highest quality of care. He’ll work closely with Brittany on her approach to challenging care, bringing his broad expertise in adult and pediatric medicine. Patients appreciate Dr. Coleby’s down-to-earth demeanor and problem-solving mindset. Together, he and Brittany are excited to offer an innovative care model that puts patients first.
